Weight Loss Surgery

Bariatric operations may contingently be divided into those that reduce the volume of the stomach or prevent entry of food into the stomach (restrictive) and those affecting the quantity and quality of absorbed food (malabsorptive). In addition, these operations affect the hormones released in the stomach and intestines, and exert an influence on appetite and the body’s ability to use up the food consumed or store it up as fat.

Intragastric Balloon

Intragastric baloon is a deflated silicone receptacle, round in shape, placed into the stomach and then filled with liquid or air to decrease the amount of gastric space. The walls of the receptacle are so designed as to stay in the stomach for a maximum of 6 months. The balloon is inserted into the stomach using endoscopy. Its positive aspects are the ease of implementation procedure, minimum level of contraindications, low complication rate and the attainment of weight loss without any significant effort on the part of the patient.

Adjustable Gastric Banding

The operation of gastric banding has been applied since 1976. A silicone band is placed just below the esophagus-gastric junction, causing the formation of a small (proximal) and a large (distal) gastric segment.The volume of the small gastric pouch, restricting nutrient intake, is only 15 ml. The food passes from it to the big gastric segment through a narrow outlet as in an “hourglass”.

Sleeve Gastrectomy

By means of special laparoscopic cutting and stitching instruments, a resection is carried out of a major portion of the stomach in longitudinal direction. In this way the stomach is shaped into a narrow tube. As a result, the amount of food intake is restricted (to 100 ml), which leads to the loss of excess weight of up to 50% on average. This operation is used both as an independent method of treatment and as the first stage of Biliopancreatic diversion.

Gastric Bypass

This is a combined bariatric operation which includes both restrictive and malabsortive components. Weight loss is achieved by the formation of a small ventricle through which nutrient gets into the jejunum. Through another loop, comprised of the duodenum and the proximal part of the jejunum, enter bile and gastric juice which mix with nutrient in the distal part of the jejunum. This reduces not only the amount of food consumed but also the absorption of nutrients and leads to a stable weight loss, along with a regression of diseases constituting the metabolic syndrome.
